What to bring?

Please arrive 15 minutes early to your first visit to fill out any necessary paperwork and change into appropriate clothing. Remember to bring the following:

• Doctor’s referral or RX(Prescription)
  
• Medication list
• Insurance card(s)
• X-ray, MRI or any diagnostic report
• All medical record(s) in relating to your injury or condition
